WHAT IS BOTOX™ AND HOW IS IT USED?

Botox is a bacterial product that is a type of neurotoxin. It is specifically administered to reduce the activity of overactive facial muscles. This activity can lead to unwanted wrinkles. Originally used to treat certain eye disorders, this protein can now be used to reduce frown lines of the nose, forehead, crows feet, and wrinkles around the mouth. In certain instances, it can also be used to reduce neck lines and sagging jowls.

HOW LONG DOES THE TREATMENT TAKE? HOW MANY SESSIONS ARE NEEDED?

Our average, treatment sessions take from five minutes to 30 minutes. Treatment time correlates with the area(s) that are treated and can be altered if we perform concomitant filler therapies. One treatment with Botox lasts from three to six months. When fillers are combined with neurotoxin, studies have shown a synergistic effect that can, in some instances, last up to 12 months. Repeat treatments are carried out when the cosmetic benefits have worn off.

HOW MUCH DISCOMFORT CAN I EXPECT FROM THE TREATMENT?

There is minimal, if any, discomfort with this procedure. No topical or local anesthesia is required.

HOW LONG DOES IT TAKE TO SEE THE RESULTS?

It can take up to seven days to see results from the Botox procedure. If you do not see results by this time, then it is possible that you are resistant to treatment or are forming antibodies to the toxin.

WHAT ABOUT POST OP CARE?

We recommend avoiding exercise for 24 hours. Do not lie flat for four hours after treatment. We also recommend that you have Tylenol™ ready in case a headache develops. Do not massage the area unless otherwise directed. Patients are advised to call us if they develop complications, such as brow drop or double vision. These are very rare occurrences and can be resolved in short order.

TELL ME ABOUT FILLERS? AREN’T THEY ALL THE SAME?

Our experience has shown that fillers have different properties. There has been such a boom of filler types that the cosmetic dermatologist can now discriminate about what to use. For example, Juvederm™ or Restylane™, which are derived from non-animal synthesized hyaluronic acid (NASHA), have a natural feel and can be used in the lip and forehead areas. Sculptra™, on the other hand, is a volumetric enhancer. It can define and elevate the cheeks and fill in the hollowed areas of the temple. Radiesse™ is a natural choice for the nasolabial folds and is used for our “Volume Lift” procedure – see below. This latter product is derived from calcium hydroxylapatite spherules. It is a wonderful filler to use because of its longevity and safety. Radiesse™ can be used to treat aging hands, the jawline, and cheek areas. Finally, we can harvest a patient’s own fat cells (Lipotransfer) to treat all of the areas mentioned above. Natural fat works well to reduce under eye circles and can improve lip volume.

HOW OFTEN ARE FILLER TREATMENTS PERFORMED?

Filler treatments are performed when the cosmetic effects have worn off or are less noticeable. On average, Juvederm™ treatments are repeated at eight to 12-month intervals, or sooner for touch up of the lip areas. Forehead lines, when combined with Botox Cosmetic™, can maintain their effects for up to a year. When using Radiesse™, studies have shown that this material can last up to two years in the nasolabial folds. Finally, Sculptra™ is usually administered in three to five patient sessions over a 20-week period. Results usually last for two years or more. This is very satisfying for our valued patients who want to get the most out of their investment.

BEFORE AND AFTER – HYALURONIC ACID FILLER TREATMENT

WHAT IS THE TREATMENT PROTOCOL?

We ask you to discontinue blood thinners, such as Vitamin E, St Johns Wort, and Aspirin, beginning one week before treatment. Prior to the procedure, we will prescribe a topical anesthetic cream. This is left in place for 30 to 60 minutes. If we are treating the lip area, our providers will perform a virtually painless dental block. After numbing has been achieved, the filler is placed into a predetermined area of your skin. Ice packs and pressure are used to minimize bruising. You can also start on Arnica before your filler treatment to minimize bruising. Treatment time takes between 10 and 45 minutes.

WHAT IS A VOLUME LIFT AND HOW IS IT PERFORMED?

The “Volume Lift” refers to a large volume filler augmentation procedure. It can be accomplished with Lipotransfer, calcium hydroxylapatite (Radiesse™) or poly-L-lactic acid (Sculptra™). Lipotransfer refers to grafting of donor fat cells from your own body. This is carried out via liposuction. Self-harvested fat cells are referred to as autologous fat and are one of the most advantageous filler substances known to physicians. Facial wrinkles, acne scars, eyelid creases, post-traumatic scars, aging hands, and dimpling of the breast have benefited from the use of the “Volume Lift.”

HOW LONG DOES THE COSMETIC BENEFIT FROM LIPOTRANSFER LAST?

The survival of transplanted fat is sometimes difficult to quantify and is variable depending on the patient’s medical status, age, and the location of the actual injection. Some studies have demonstrated that fat graft survival may last up to 12 months. Repeat treatments tend to create a longer-lasting cosmetic benefit.

WHAT ABOUT THE AFTERCARE?

Filler treatments may induce some degree of swelling or bruising with each procedure. Ice packs and/or pressure can minimize this unwanted complication. After your procedure, it is important that you massage the treatment areas using the “3-3-5” rule, e.g., massage the treatment area three times a day for three minutes for five days. Bruising, when it occurs, usually resolves within a matter of days.
