WHO BENEFITS FROM HAIR TRANSPLANTATION?

Hair transplantation is a great surgical technique for those who suffer from hair loss. Both men and women can benefit from this technique. Hair transplantation is most commonly utilized to treat male pattern baldness. It can also be used to restore eyelashes, eyebrows, facial and chest hair as well as to fill in scars caused by accidents or surgery such as face-lifts and previous hair transplants. This procedure differs from other types of dermatology procedures in that the hair grafts contain almost all of the epidermis and dermis surrounding the hair follicle.

Both strip harvesting and follicular unit extraction (FUE) can be utilized as hair transplant techniques.

Strip Harvesting

A strip of scalp is removed under local anesthesia and the wound is then sutured back together. The donor scalp tissue is then cut into small pieces of tissue called grafts which are then transplanted back in to the donor (thinning area) site of the patient’s head. This approach leaves a linear scar in the donor area. Sutures are removed around 2 weeks after the procedure is performed.

Follicular Unit Extraction

Follicular Unit Extraction (FUE) is a procedure that removes individual follicles of hair under local anesthesia. Each follicular extraction removes a small punch which measures between 0.6mm and 1.25mm in diameter. Each follicle is then reinserted back in to the scalp in the thinning area using a micro blade. Because individual follicles are removed, only small scars remain and post-surgical pain is minimized. As no suture removal is required, recovery from FUE takes only about 7 days. Some surgeons note that FUE has a lower success rate compared to strip harvesting. Others suggest that if a second transplant is needed after FUE, the donor site may be less reliable.

WHAT HAPPENS DURING THE PROCEDURE? WHAT CAN I EXPECT?

At an initial consultation, we will analyze the area of hair loss, discuss your preferences and expectations, and advises you on the best approach to take (e.g. single vs. multiple sessions) and what results might reasonably be expected.

For two weeks prior to surgery, you should discontinue medications which can cause intraoperative bleeding and/or poor “take” of the grafts. Alcohol and smoking can contribute to poor graft survival. Post operative antibiotics are usually prescribed to prevent surgery infections.

Hair transplant operations are performed on an outpatient basis, with mild sedation if desired and tumescent local anesthesia. Tumescent anesthesia lasts for six to twenty-four hours. The scalp is shampooed and then treated with an antibacterial agent prior to the donor scalp being harvested. The strip harvesting or follicular unit extraction (FUE) procedure is then performed.

WHAT HAPPENS IN THE POST-OP PERIOD?

A semipermeable dressing is placed which allows for the seepage of blood and tissue fluid. This dressing is changed on a daily basis. The recipient area must be protected from sunlight and trauma, and shampooing is not begun until two days after the surgery. Shampooing is important to prevent crusting from occurring around the hair shaft. These crusts adhere to the hair shaft and increase the risk of losing newly transplanted hair follicles.

During the first ten days, virtually all of the transplanted hairs will fall out. This is referred to as “shock loss”. After two to three months, new hair follicles will begin to grow from the moved grafts. The patient’s hair will grow normally, and continue to thicken for the next six to nine months. Subsequent hair loss is likely to occur from those sites not treated by surgery. Some patients elect to use medications, e.g. Rogaine™ or Propecia™, to retard such loss, while others require a subsequent transplant procedure to deal with remaining hair loss.
